After a team evaluation, a thorough report is written providing:

• Specific recommendations
• Accurate costs
• Funding recommendations
• A plan of action
• Medical justification
• A prescription

The report is used by the Hospital or the client's guardian to obtain funding for the recommended equipment. The equipment can be obtained, adapted and fitted by the Hospital directly or with the client's own choice of vendor.
