Children's Specialized Hospital Long Term Care
Assessment and Admission
Our Pediatric Long Term Care services at Children’s Specialized Hospital Long Term Care Centers in Mountainside and Toms River, New Jersey are offered to medically fragile children whose family’s circumstances no longer allow them to be cared for in the home and/or other community settings.  Examples of children we serve include: children with feeding tubes, tracheotomies, vent dependency, and other medically complex needs.

Admission to our services is limited to children ages 17 and under and requires a review of the child’s medical, psychological and developmental needs along with documentation of immunization, health history and social history to determine appropriateness.  We invite families to tour, review our family guide and admission agreement to assist them in their decision making process.  Federal regulations also require all applicants to a Medicaid-certified nursing facility be screened through the Preadmission Screening and Resident Review (PASRR) process.  These steps along with financial clearance are all needed prior to any admission approval.

Our Admission Director and team are happy to discuss the referral and admission process in more detail along with information about our day to day care at our centers.  A brief description is available to you via our website www.childrens-specialized.org by clicking on the Programs and Services tab and then Long Term Care. You can also feel free to contact me for any questions or information requests that you may have.

I am certain that your days are filled with challenges in helping the children and families of our state who have special needs.  Our hope is that we can be a resource to you when and if these services are the answer.
